What Health Programs
Can Do

  • Help you understand the basic pathophysiology of health problems
  • Describe risk factors and safety issues to you
  • Screening and assessment
  • Describe physiological changes (i.e. pregnancy and childbirth)
  • Assist with medication management
  • Life skills training and education
  • Demonstrate proper use of related health tools such as glucose meter or peak flow meter and more
  • Teach you how to document related activities such as diabetes log book, asthma diary, weight management log and more
  • Set self-management goals with you and teach self-monitoring techniques
  • Follow-up with you via phone call, home visit or group sessions to assist with the barriers of living a healthful life
  • Facilitate chart review
  • All services available in Spanish
Our Health Educators and Community Health Workers help you complete the patient health care cycle by giving you the personalized, in-depth information you need to follow the health plan prescribed and bilingual services are available.
We offer many classes for weight management, incl. weight management for diabetes and healthy heart care, as well as for prenatal care and reproductive health, smoking cessation and more.
